While most parents stick to their naming decision, there are moments when doubt creeps in — maybe the name is constantly mispronounced, or perhaps it carries a meaning that doesn’t feel right.

Well, if you ever find yourself in this situation and wonder whether it’s possible to make that change after the birth certificate has already been issued, the answer is yes, but it may not be as straightforward as you might think.
Changes can be made for children less than a year old
For cases where the child is below one year old, a request to amend the name can be made at any Jabatan Pendaftaran Negara (JPN) outlet across Malaysia.
The amendment costs RM5. Do note that only the biological mother or father of the child, or a legal guardian (with a letter of permission to care for the child from JKM), can file for the amendment.
Other documents required for the process include:
- Application form for name entry/correction of information (JPN.LM21) (original)
- Child’s birth certificate (original)
- Mother’s and father’s MyKad (original and copy)
- Declaration form (AM80) for parents (original)
- Family information form (BMK 81) (original)
- Parents’ marriage certificate (if applicable)

Can you file for a name amendment if the child is more than one year old?

The short answer is — yes. You can still apply to have your child’s name amended even if they are over one year old. However, a few additional steps are required before the application can be made.
First, name changes can only be made based on certain conditions. For example, if the chosen name carries a negative meaning.
Next, parents of a child over one year old must provide supporting documents from one of the following:
- A court order
- A letter from the Islamic Religious Department or the Mufti’s Office (for Muslim children)
- A letter from a religious body (for non-Muslim children)
- A letter from the embassy (for children with non-citizen status)
Additionally, letters of support from these parties — excluding court orders — must include information on the meanings of both the existing name and the proposed new name.
Do note that applications are limited to one amendment only. To ensure a smooth process, parents are advised to submit complete and accurate information.
